On The Mend

That this House welcomes the launch of the report On the Mend: Hospital Discharge Services and the role of Home Improvement Agencies, published by Care and Repair England; commends the excellent services highlighted in the report which are helping older and disabled people return to their own homes after a stay in hospital; recognises that On the Mend clearly demonstrates the importance of improving the links between housing, health and social care if the Government's aim of enabling more older and disabled people to live independently in their own homes is to be achieved; commends this report; and calls upon the Department of Health and the Department of Transport, Local Government and the Regions to work together to encourage more local housing authorities, social services authorities and health sector planners and commissioners to work together to develop further hospital discharge services such as those highlighted in On the Mend.
